Rationale
Need for modification brought about by circumstances which a diligent contracting authority/entity could not foresee: The appointment of an alternative provider would have resulted in a risk of disruption to patient care. The modification therefore represented the most efficient and proportionate means of meeting the urgent service requirement whilst maintaining service continuity, patient safety, and value for money, whilst a process is undertaken for the longer term contract.
