The service delivers comprehensive initial health assessments, vaccination and immunisation support, ongoing clinical care for complex health needs, and timely signposting to appropriate NHS and community services. The team supports early identification of physical and mental health needs, facilitates registration with local GP practices, and works to reduce barriers to accessing mainstream healthcare, helping to prevent avoidable deterioration in health and unplanned use of urgent care. The contract will include administrative elements to support continuity of service provision until strategic review of commissioning arrangements has been concluded and new contract put in place (March 2027), following exit of previous provider (Norfolk County Council). This is a 12 month contract from 1st April 2026 to 31st March 2027 with a total value of £144,000 with no option for further extension.Value not stated
