The service will include a range of interventions organised into three core pillars: Pillar 1: a universal self-help pillar comprising of health promotion campaigns and digital self-help tools Pillar 2: a behaviour change pillar providing health coaching support to address the in-scope risk factors Pillar 3: a structured treatment pillar targeting reducing inequalities through provision of community falls prevention, an enhanced behaviour change support, and a targeted NHS Health Checks programme in workplaces and specific communities.
