We will be using a number of engagement methods to work with the community health and well-being (community homecare) providers in Leeds to understand their views on our plans and work in partnership to hopefully develop a solution that will benefit citizens in Leeds to receive good quality, outcome-focussed, person-centred care, at the point of need.
