Shropshire Council, in partnership with the ICB, is seeking to commission a new preventative services contract aimed at supporting Shropshire residents aged 18 and over. This contract will adopt a preventative approach, delivering a variety of place-based, community-led initiatives designed to promote healthy lifestyles, address inequalities, and provide early interventions that enhance residents’ health and well-being. In doing so, it aims to reduce reliance on statutory services. The Local Authority intends to structure the contract around three themed lots, with an emphasis on strong collaborative working. Description
The contract holder will deliver countywide provision for people when they are discharged from hospital or at risk of an unplanned admission. The provision will involve a variety of support including transportation where needed, alongside practical and emotional support that will aid an individual’s recovery and reablement, ensuring that health, wellbeing and independence is maximised, and the risk of readmission and/or carer breakdown is minimised. This will include but not be limited to: food shopping/prescriptions collection; access to low level aids, equipment and assistive technology and connectivity to sustained support that will encourage healthy lifestyles and strong social networks. This provision will maintain a focus on both the needs of individuals being discharged and their family carers. The maximum annual value for this lot is £200,000.
