The implementation/addition of a Pulmonary Rehabilitation (PR) Platform developed by AHP Toolkit Ltd. The Web platform will provide a trusted source of PR information to the population of Norfolk and Waveney, including courses details, location and availability, eligibility criteria, self-referral option, COPD treatment and symptom management, Patient PR education, Post PR Course advice and guidance. Information will be available in multiple languages. Research has identified this will: Reduce hospital admissions Reduce inpatient hospital days Reduce re-admissions Reduce the number of home visits Improve health related quality of life Patients better informed Be highly cost-effective This modification does not alter the length of the original contract and the change to the lifetime value of the contract will be an additional £38,500 rendering the new lifetime value of the contract £872065843 The effective date of modification is to be confirmed
